Insurance does not cover ketamine infusions. If your insurance is contracted to
cover the charges for the office visit (usually CPT codes 99215, G2212), this benefit
will be deducted from your amount due. It is YOUR responsibility to notify Happier
You immediately if your insurance coverage lapses or changes. Rejected claims will
be your financial responsibility.
Happier You is currently contracted with Anthem Blue Cross Blue Shield and CIGNA for in-network visits.
If your insurance is not contracted in any way with Happier You, you may request a
superbill to make your own claim or request we apply for out-of-network benefit using
Reimbursify (if applicable). We will not negotiate with the insurance company on your
behalf.
